If you have dental insurance which covers your treatment and would like us to file your claim, please complete all information on the patient registration form so that the claim can be processed. We are preferred providers with the following insurance companies:

• Value Care
• PEHP
• Met Life*
• Select Health*
• Delta Dental*
• Dental Select*
• Cigna*
• Aetna*

*Not offered by all practice Periodontists. Please inquire when scheduling your initial appointment.

Please keep us aware of any changes to your insurance plan so that we can keep our estimates of coverage as accurate as possible.

Keep in mind that even if a claim is in process and hasn’t been completed, your portion (co-pay, etc) of the costs are due at the time of service.

If the insurance company rejects a claim, we will assist in negotiating the claim. However any unpaid balance is ultimately your responsibility regardless of the claim resolution.
